1. Visit or call the parish office. 
2. The parents must be registered parishioners of San Martin de Porres or the parish missions (Guadalupe or St. Jude), or live within the parish boundaries.
3. The parent must present the child's original birth certificate (long form) from the city or the state of birth.
4. The church will not accept copies or faxes of the child's birth certificate.
5. Parents are required to complete a Baptism class in order to baptize. The godparents also need to attend the class.
6. The parents pay a donation fee at the time of registration.
7. The parents pick up forms of Baptism for godparents.

2. Los padres deben ser feligreses registrados de San Martín de Porres o de las misiones parroquiales (Guadalupe o San Judas Tadeo), o vivir dentro del territorio de la parroquia.
3. El padre debe presentar el certificado de nacimiento original del niño (forma larga) de la ciudad o el estado de nacimiento.
4. La iglesia no aceptará copias ni faxes del certificado de nacimiento del niño.
5. Se requiere que los padres completen una clase de Bautismo. Los padrinos también necesitan asistir a la clase.
6. Los padres pagarán un donativo en el momento de la inscripción.
7. Los padres recogen formas de Bautismo para los padrinos.
    

A sponsor also helps the baptized person to lead a Christian life in keeping with baptism and to fulfill faithfully the obligations inherent in it (Canon 872).

El padrino ayuda al bautizado a llevar una vida cristiana conforme al bautismo y a cumplir fielmente las obligaciones inherentes a él (can. 872)

CHOOSE A PERSON WHO IS ...

• Baptized Catholic who received their First Communion and Confirmation (Canon 874 §1)
• Have completed the sixteenth year of age (Canon 874 §1)
• A Catholic who leads a life of faith in keeping with the function to be taken on (Canon 874 §1)
• Not the father or mother of the child (Canon 874 §1)
• Not be bound by any canonical penalty legitimately imposed or declared (Canon 874 §1)

► If married, the person or couple need to be married in the Catholic Church. Those who do not participate regularly in the sacramental life of the Church, especially in the Eucharist, and/or who are living in an irregular marriage (civil marriage) do not qualify to be godparents or sponsors.

• Haber cumplido los dieciséis años de edad (Can. 874 §1)
• Vivir una vida de fe de acuerdo con la función que debe asumir (Can 874 §1)
• No puede ser el padre ni la madre de la persona (Can. 874 §1)
• Ninguna pena canónica legítimamente impuesta o declarada (Can. 874 §1)

► Si es casado, la persona o pareja debe estar casada en la Iglesia Católica. Quienes no participen regularmente de la vida sacramental de la Iglesia, especialmente de la Eucaristía, o viven en matrimonio irregular (matrimonio civil), no califican para ser padrinos.
